GET HEALTHY VI is an incentive-based wellness program design with monthly challenges to help you meet your long-term health and nutrition goals. Every monthly challenge will have a theme and you will be given weekly tasks to complete through our online platform for entry into our grand prize raffle. These tasks are designed to help you start making positive lifestyle changes. As a member, you will receive many other benefits outlined below.

Our Mission is to empower participants with health education, lifestyle skills, & a support system that enable them to achieve their best possible health & to engage with local businesses that promote a healthy lifestyle.
To enroll in Get Healthy VI you must complete an initial 60 minute assessment with our Registered Dietitian Nutritionist (RDN). Appointments can be made at either Caricare Family Health Services or Synergy Fitness and Wellness Center. With your initial enrollment comes unlimited access to our Practice Better tracking program, entry into your first monthly challenge and access to purchase future challenges.
Insurance payment for initial enrollment is accepted if your insurance covers nutrition counseling. If you attend a one-on-one follow-up appointment during any subsequent month, you will be automatically enrolled in the program for that month.
Every month (STARTING OCT 2019) we will have a raffle for a Grand Prize worth anywhere up to $500. Raffle Prices subject to change each month. Participants will receive one entry for every task you complete in Practice Better. In addition, you will receive one entry into the raffle for each Get Healthy VI event you attend.
In addition to our grand prize, the 5 participants that receive the most raffle entries will win free entry into the next month’s challenge.
Online Support with Practice Better
Enroll in each monthly program through our secure online & mobile platform, Practice Better, for easy tracking, feedback, and access to challenge materials. You will receive an email to sign up after your initial enrollment and will be automatically enrolled in the next challenge. You will have to sign up for subsequent challenges, which can be done through Practice Better.
Each week of a challenge you will receive emails with challenge details, education materials and notifications for tasks to complete. Track your food, activity and other habits through our platform to receive raffle entries and feedback from your Registered Dietitian Nutritionist. You can also send messages directly to your RDN.
Get Healthy VI will offer monthly events to help keep you on your health and wellness journey. Location, time, and type of events will vary each month.
- Monthly Support Groups: Each month we will host a Nutrition Class at each location related to the challenge topic where participants can engage with our RDN and other participants. Plus, track health data, such as weight & blood pressure.
- Special Events: We will also host special events to help our participants engage with healthy businesses in the community. Examples include fitness classes, grocery store tours, cooking classes, and guest speakers. Special events may be open to the public for a fee. In addition to the benefits listed above, members will receive discounts on goods and services for supporting our healthy business partners on the island. You will receive a list of benefits upon enrollment.
